Your Right to a Good Faith EstimateExpected-charge information for uninsured and self-pay patients.
Over the Knife is a self-pay service. Uninsured patients and patients who choose not to submit a claim to insurance have the right to receive a written Good Faith Estimate of expected charges when health care services are scheduled or requested.
The estimate describes the expected service and charges. It is not a contract and does not require you to receive the service. Good Faith Estimates are provided through the secure scheduling process and are also available upon request.
Cancellation and ReschedulingNo cancellation penalty. A $500 fee applies only to a missed appointment without cancellation.
Appointments may be canceled or rescheduled without a cancellation penalty before the scheduled start time. Please provide notice as soon as possible so the reserved appointment can be released.
A $500 missed-appointment fee applies only when a patient does not cancel and does not attend the scheduled consultation.
The fee may be waived for a documented emergency, a service-side technical failure, or another exceptional circumstance at the discretion of Dr. Kwon, DC.
